Entrants may enter the contest by fully completing and submitting the contest entry form at

CONTEST PERIOD
The contest will begin on May 11, 2011 and ends at 11:59PM, PST, on June 1, 2011. Winner will be chosen by random drawing among all eligible contestants. All winners will be subject to verification and must meet all eligibility requirements before a prize is awarded. Prize winner will be notified by email, telephone or US mail within thirty (30) days of contest end. If a winner is unreachable for five (5) days after being contacted by DolphinStreet, or if that winner is unavailable for prize fulfillment, potential winner will be disqualified and an alternative winner will be selected by random drawing among all remaining contestants.
PRIZE
One (1) Lex Rotary effects pedal, $299 value. The odds of winning depend on the number of eligible entrants. Prize is awarded with the standard factory 1-year warranty. No substitutions, cash equivalents, or transfer of the prize is permitted. Prize will be shipped to winner at no cost. Winner is responsible for the payment of all applicable federal, state and local taxes, fees and duties.
